Data Team Action Template: This artifact is a copy of one of the action templates my data team completed. In completing this template and process, we first selected a standard to meet and wrote our learning target based upon that standard. All of the remaining steps were based upon student achievement on a pretest along with later observations and formative assessments. Instructional focuses and strategies were selected for each group of students based on their assessed needs in relation to the standards.
Student Data Organization Form: This form is one I created for use with our math curriculum during the 2012-2013 school year. This served as a guide for planning instruction based on student achievement data. The initial boxes were completed after looking at student performance on a pre-assessment for the unit of study. Recognizing proficiency before the lesson allowed for more differentiation within the lesson as students were grouped based on need. Following the lesson, I examined student performance on a ‘quick check’ given at the end of the lesson. Based on student scores on this formative assessment I planned the next steps for instruction. This included both differentiated homework practice and needs based groupings during math workshop.
