Organization Template: The first artifact I have included as an example of how I use student performance data as a guide for decision making is an organization template I created to use with our new math curriculum. The pre-assessment data identifies who is already proficient for each lesson and I use that data to determine groupings for each lesson. Students who are already proficient are allowed to work practice problems independently, while those who scored farther from proficiency meet with one of the teachers in a smaller group during the lesson to receive more individualized instruction. At the close of each lesson students complete a Quick Check (formative assessment). There is space at the bottom of the attached template to record student scores from this assessment and note the next steps needed by each group of students during the next day's math workshop. This organization allows both my co-teacher and I to be confident that we are meeting the needs of each learner in our classroom.
