Behavior Report Card: This artifact is a behavior report card I created to use with one of my students. Aside from evidence of student's academic learning, I also provide evidence of student's social and behavioral learning using point sheets similar to this one. I used three point sheets with six different students during my first year of teaching as a way to monitor behavior, provide opportunities for positive reinforcement, and communicate with families. The student and I give a score for each goal each day and I frequently write comments explaining the points. These sheets go home on Friday's to parents and are brought back signed on Monday with any comments or questions a parent may have had. Aside from the weekly feedback to families, I have also graphed the points to show parents at conferences, utilizing the data from this one item to provide a second form of evidence of growth.
Fluency Graphs: A second example of how I provide evidence of student learning and growth to others can be seen in the fluency graphs I have attached. My co-teacher and I do 1-minute running records on students approximately 2 times a month (some students are tested more often, especially if we are reconsidering reading levels). This data is graphed to create a record of student progress and an easy visual to share with one another, with students, and with parents at conferences. Aside from just student scores, I have also included goal lines for each area plotted on the graph - reading level (using DRA score), words correct per minute, and accuracy. This helps students see not only where they are, but where they are working to get to.
