I added the investigation Death by a Thousand Clicks: Where Electronic Health Records Went Wrong to the e-Iatrogenesis page. A joint investigaton by Fortune and Kaiser Health News says "Inside a digital revolution gone wrong. E-iatrogenesis is just the most concerning because it is hard to detect. However, the user experience can be typified by their statement "Critical or time-sensitive information routinely gets buried in an endless scroll of data, where in the rush of medical decision-making—and amid the maze of pulldown menus—it can be missed."
Data entry gets shifted to the physician for the good of "the system" meaning the benefit of their payers. My primary care physician spends 7-10 minutes refilling my chronic medications on an EHR with much of the time spent adding the diagnostic codes. Great for the insurance company as they can approve or reject prescription claims within a minute. Not so great for the patient who watches the physician scroll through endless menus.
