Forms

Please mail all required forms to the registrar as listed on the bottom of the Family Contract. Mailing these forms with your registration fee within 5 days of registering will secure the classes you have selected for your student.

(Printing double-sided is fine.)

• Release Form-Waiver   (Print all 4 pages below & complete for each student. Page 4 "Signature of Releasors" shows acceptance of the entire four page Permission Form, Waiver, and Release document and should be signed by both parents.

• ASSG Policies, Procedures, & Family Contract   Read all of the pages with your student(s)!  Parent responsibilities, student responsibilities, student dress code, and the plagiarism policy are included and should be well-understood.  Sign as indicated on the form and mail the final signature page only. You may print pages 1-5 for your own reference of the policies & procedures.)
