Step 1: Download and fill out the request form (see below) 

Step 2: Mail completed form, along with $5 CASH/CHECK payable to: Shorewood High School

Step 3: Address Envelope:

Step 4: Make sure the envelope makes it to the post office!

Please note, we do not retain copies of high school diplomas, ACT, SAT, or AP scores.
