Suarez Express Scholarship Program through DSWD's Assistance to Individuals in Crisis Situations (AICS)
ALL COLLEGE CORDIANS RESIDING IN THE 2ND DISTRICT OF QUEZON PROVINCE (Lucena City, Sariaya, Candelaria, Tiaong, Dolores, San Antonio) ARE ELIGIBLE FOR THE PROGRAM
Beneficiaries of CHED UNIFAST, 4Ps, and other Government Scholarships programs are subject to assessment.
PROCESS OF APPLICATION:
A. Visit the SHC Registrar and Scholarship Office starting April 11, 2023 to get the APPLICATION FORM and CERTIFICATE OF ELIGIBILITY.
B. Submit the filled-out forms together with the following requirements:
Printed Statement of Account from the PORTAL certified by the Finance Officer.
Xerox copy (back to back) of School ID with 3 specimen
Barangay Indigency with the note "For Educational Assistance Application purposes".
Copy of Grades with GWA (First Semester) of at least 85%, issued by the Registrar's Office.
1pc of 2x2 picture pasted in the application form.
C. Interview and assessment by the Scholarship In-charge/staff. Once qualified, wait for the schedule of PAY-OUT.
