Awarded to the dependents of non-combat casualties (killed-in-action or KIA and deceased/complete disability discharged) and dependents of active military personnel.

REQUIREMENTS

• Accomplished AFPEBS Application Form with (two) 2 copies of latest 2x2 picture of the applicant

• Authenticated clear copy of:

(1) applicant's birth certificate and Form 138 (Report Card); (2) marriage contract of parents;

(3) parent's death certificate, casualty and posthumous  report (if applicant is the child of military personnel killed-in-action (KIA))

• Certificate of good moral character from last school attended
