Form Responses can be checked through different options available for IP / IP Agent and Samhita / CGF User. For the Samhita / CGF User the process is as follows:
Process 1:
Navigate to Forms List, search for the relevant form and click on the three dot options and click on "Responses"
Process 2:
Navigate to Projects List, search the Project, open the project by clicking the name
Scroll down to the Data collection section and search for the relevant form
Click on the three dot options and click on Responses
Process 3:
Navigate to the Form Responses option and search for the Form associated with the Project, and click on the three dot options, and click Responses
The Form Responses will be shown in a tabular format with the questions being the columns and rows capturing the responses. The responses can also be downloaded in csv format as well.
Form Responses can also be uploaded in bulk as well. This can be done when navigating to the Form Responses of any specific form of the Project. On the Form Responses table, there will be a button called "Bulk Upload" which when clicked will open the page to upload a template file against the specified form. Every form created has different fields so the template will also be dynamic based on the form.
But before we can download the form bulk upload template, we have to set certain parameters with the options as described below:
Implementation Partner Name: Here the IP Name has to be selected if a Samhita / CGF User is accessing it. In case of an IP accessing this page, the Name will be of the same IP who is accessing the page.
Agent Name: Under the selected Implementation Partner, all the onboarded agents names will be shown and any one of them can be selected. This is an optional field but this will help in filtering the data.
Unique Identifier: The unique identifier will be utilized to appropriately map the right list of participants with the Form Responses. There are two types of unique identifiers that can be used:
Participant Unique ID: The unique id of the participant added by the IP / IP Agent during onboarding will be used as a reference.
Participant Mobile Number: The mobile number can also be used as a unique identifier to map the participants to the form responses.
Date Parameter: The Date Parameter will be used to filter the list of participants based on the Participant Onboarding Date.
Once all these parameters are added, the downloadable template will be available for download.
The template once opened will have the rows of Participant details like: Participant Unique ID, Participant Name, Participant Mobile Number, Participant ID added by the IP and followed by all the columns which will have the form fields.
The Participant Unique ID created by the Samhita App will be auto populated, the Participant Unique ID added by the IP will also be auto populated but the mobile number has to be entered manually by the User which will be matched with the Samhita Unique ID.
Once all the successful validations are done, the responses will be uploaded against the selected participants.
In the Samhita Platform once the Project is created, and the enrollment form is also created and published then the IP / IP Agent on behalf of the participant, will be able to fill the form and enroll the participant.
Once this process is done, and if there are different stages of data collection that needs to be fulfilled then the Samhita / CGF User will create the relevant form for the Survey and publish it. This published survey form will be accessible to the IP / IP Agent through the following process:
Navigate to Participants > Onboard Participants
Search for the required participant and click the three dot options and click on Edit.
Go to Project Details, it will show the list of Projects that the participant has been enrolled for.
Click on the three dot options against the required project, and click on the Custom Forms or Survey Forms option. If Custom Forms option is selected then the published custom survey form can be selected and filled on behalf of the participant. If the Survey Forms option is selected then the relevant SurveyCTO form created at SurveyCTO will open and can be filled on behalf of the participant.
