Once a Project is created and Save Project is clicked, a new section will appear on the Same Page called "Data Collection". This section is where, the Samhita / CGF User can create Forms, Surveys or Questionnaires which are required as data collection points for the Project. There are 3 ways in which a Form can be created:
Start from Scratch:
The start from scratch is an option that can be used to create a Form using the Samhita's Form Builder, and then publish this form to the assigned IP for them to utilize this form for enrolling or capturing responses of participants. When the Start from Scratch option is selected, the Form Builder opens which has majorly 4 sections:
Basic Details Section:
In this section, the Form Name, Form Description, Form Status (Published / Unpublished), and selecting the Deliverable / Activity / Task to which this forms needs to be mapped to.
Form Canvas:
The Form Canvas is where the Form Elements can be dragged and dropped into and the form can be created.
Form Elements:
In this section, different form elements can be used to create the form. These form elements can be dragged and dropped into the form canvas. Following are the form elements available:
Heading
Name
Phone
Date & Time
Address
Paragraph
Dropdown
Single Choice
Multiple Choice
Number
File Upload
There is also a Customize Form option that will enable us to edit properties of the Form or the Page background.
Form Properties:
Form Properties cover two aspects:
Form Elements Properties:
These properties are specific to form elements that have been added into the Form Canvas. These properties would include some alignment properties, the required property which basically mandates a field to be entered, label, sublabel and placeholder properties, text size properties and some other aesthetic level properties also.
There is also options to View the created form in Desktop or Mobile Mode. Form can be published using the Published button which will help IPs to capture responses of Participants by filling this form.
In case if the Form needs to be worked on later then the Save to Draft option can be selected which will save the form to be edited later.
Form / Page Properties:
The Form / Page Properties can be accessed by clicking Customize Form which will show the Form Properties like changing the background color or changing the background image or the same properties apply for the Page background as well.
The Samhita Platform has integrated with the SurveyCTO Platform to help Users create forms at SurveyCTO and publish the same under the Samhita Platform. After creating the project or while updating the project, in the data collection section and click on the "Create Form at SurveyCTO" option, which will redirect to the SurveyCTO Platform. The form can then be created at SurveyCTO and once the form is saved, it will redirect back to the Samhita Platform.
The form then can be published against the Project and IP can fill responses on behalf of the Participant.
Samhita Platform also provides the option to utilise already published forms and then modify it as per requirement. Once a Project is created or is updated, in the data collection section, there is a Forms List which shows existing Forms available in the Platform that were published.
There are a few options available here:
Edit: This option is only for those Samhita / CGF Users who have created the form. This option opens the form in the Form Builder and allows to add new Form elements to the existing ones. The existing form elements cannot be removed.
Duplicate: This option can be utilized to duplicate a specific form and modify it as per requirement. Existing form elements can be edited or removed as well.
Preview: This option will preview the form
Responses: This option will show the responses captured against a Form.

Disable:
