This section contains the required forms and working documents used throughout the student teaching experience. These forms support communication, documentation, reflection, and verification of program requirements. Collectively, they ensure consistency across placements and provide a clear record of the teacher candidate’s progress and completion of expectations.
Mentor teachers, college supervisors, and teacher candidates will reference these documents at various points during the placement. Some forms are completed on an ongoing basis, while others are finalized at specific milestones or at the conclusion of student teaching. All required documents must be completed accurately, maintained in the Student Teaching Notebook, and submitted as directed.
The Working Documents section serves as both a practical resource and an accountability tool, supporting effective supervision, reflective practice, and compliance with institutional and credentialing requirements.
Midterm Evaluation
The Midterm Evaluation serves as a progress check and should occur at the midpoint of the student teaching placement. A conference date is scheduled in advance with the college supervisor and mentor teacher. Evaluation forms are distributed and completed at least one week prior to the conference. It is essential that the correct evaluation form is used, either the Multiple Subject form or the appropriate Single Subject form.
Participants are encouraged to use the TPE Rubric as a reference when completing the evaluation. During the conference, the teacher candidate, mentor teacher, and college supervisor discuss the evaluations, address questions or concerns, and identify areas for continued growth. All three participants sign each evaluation form at the conclusion of the conference, and the completed evaluations are returned to the Student Teaching Notebook.

Final Evaluation
The Final Evaluation is completed near the conclusion of student teaching. Approximately two weeks before the end of the placement, the teacher candidate schedules a final wrap up conference with the mentor teacher and college supervisor. Evaluation forms are distributed and completed prior to the conference, using the appropriate Multiple Subject or Single Subject form.
The Final Evaluation reflects the candidate’s growth since the Midterm Evaluation and overall readiness to enter the profession. Participants should be prepared to discuss the rationale for their ratings using evidence from the student teaching experience and the TPE Rubric as a guide. At the conclusion of the conference, all evaluations are signed by the teacher candidate, mentor teacher, and college supervisor. The completed forms are returned to the Student Teaching Notebook as part of the official record.
