How do I schedule a Preventive Care Wellness Visit via the VHA?

When scheduling a Preventive Care Wellness Visit, specify that you are part of the Valley Health Alliance (VHA) health plan, and a Pitkin County employee/spouse/legal partner if using a VHA provider. If you use a VHA Primary Care Network provider, you’ll receive the greatest health plan benefit.

At your Preventive Care Wellness Visit, you'll have your blood pressure taken, height and waist circumference measured, and you may have some blood tests. Your Primary Care Provider (PCP) will complete a physical exam and make recommendations regarding your general health that usually focus around healthy eating and active living, or disease screenings and well-being.

Annual Wellness Exams are free to you; covered at 100% with no copays or deductible. Your PCP will review and recommend what labs are ordered that may include: CBC w auto, A1c, TSH, Chem13, PSA, Cholesterol, HDL, LDL, Triglycerides, and Glucose. These labs will be paid for by Pitkin County, and not processed via your health insurance plan when taken at an Aspen Valley Hospital (AVH) or MidValley Family Practice lab. If these labs are drawn at a lab other than an AVH or MidValley Family Practice lab, you may be exposed to additional out-of-pocket costs.

VHA HEALTH SCREENING/LABS FEEDBACK FORM

Visit the VHA website and complete the Feedback Form to report an issue, and provide feedback on the process for free labs via the VHA. This form may be completed by employees and/or their County insured spouse or legal partner.
