Short Term Disability (STD)

MCPASD offers short term disability insurance through Madison National Life Insurance Company. The coverage is offered to employees only. 

Eligibility and Enrollment

Regular (not temporary) employees are eligible for short term disability insurance if they

  • Are currently expected to work 30 hours per week or more


Please review information STD Enrollment Packet, complete the form(s) and return to a Benefits Specialist.  

See instructions on how to submit an Evidence of Insurability form online. 

Note: You will need to complete the Evidence of Insurability medical questionnaire if 

  • You are applying for a $357 benefit level or higher 

  • You are applying more than 30 days after your first day of employment

  • You are applying to increase coverage more than 30 days after becoming employed


Short term disability insurance is offered 

  • Within 30 days of hire, if hired into a benefit eligible position

  • Within 30 days of a change in assignment to a benefit eligible position or increase in hours to meet eligibility

  • STD is not offered at open enrollment

Coverage is effective first of the month following the start/change date. 


Rates

Information concerning payroll deductions

  • Per payroll deductions are calculated by totaling costs divided by number of paychecks an employee receives annually

  • Premiums are billed the month before coverage (i.e. September deductions pay for October coverage)

  • New enrollees may have higher deductions for a time to catch up to billing cycle

  • Recalculations may happen for a variety of reasons, including change in FTE, missed paychecks or audit findings

Plan Information

Provides the elected weekly benefit during disability following 0 days for accidental injury or 3 days for health condition (including birth) or illness.

Claim Information

It is important to notify a Benefits Specialist if you believe you will qualify for short term disability benefits due to an accident, illness or condition.

File a Claim

A claim should be initiated within 30 days of the date the Elimination Period ends, if that is possible.  The claim packet is in 3 parts.

  • Employer (MCPASD Benefits Specialist completes)

  • Employee, and

  • Attending Physician
