Middleton-Cross Plains Area School District offers health insurance through Dean Health Plan. We are able to offer eligible employees a single or family level of coverage with a choice of a HMO (Health Maintenance Organization) plan, a POS (Point of Service) plan, or a HDHP (High Deductible Health Plan ) with HSA.
Employees that carry MCPASD insurance and their covered dependents are welcome to use the MCPASD Employee Health Clinic
An employee is eligible for health insurance if they
Currently qualify as ACA Full Time under the measurement method selected by MCPASD (ALE)
Are currently expected to work 30 hours per week or more
Dependent Eligiblity

Legal Dependents up to age 26
To enroll, please complete a Benefit Enrollment Form and return it to a Benefits Specialist
Health insurance is offered
Within 30 days of hire, if hired into a benefit eligible position
Within 30 days of a change in assignment to a benefit eligible position or increase in hours to meet eligibility
Within 30 days of a HIPAA qualifying life event (See Life Changing Events page for more information)
Annually during open enrollment in fall for coverage beginning on January 1
Coverage is effective first of the month following the start/change/life event date.

An employee that does not complete annual HRA will pay additional 3% rate differential of HMO premium
Information concerning payroll deductions
Per payroll deductions are calculated by totaling costs divided by number of paychecks an employee receives annually
Premiums are billed the month before coverage (i.e. September deductions pay for October coverage)
New enrollees may have higher deductions for a time to catch up to billing cycle
Recalculations may happen for a variety of reasons, including change in FTE, missed paychecks or audit findings

What is this?
Research shows that connecting with a health care provider annually and being up to date on age/gender appropriate screenings contributes to a happier, healthier life. To help support the well-being of MCPASD employees and their spouses, we offer a 3% rate differential on health insurance premiums if the HRA process is completed.
Last year, we began offering the rate differential when MCPASD employees, only, completed the process. To receive a rate differential for the 2026 health insurance plan year, spouses must complete the process. Our intention is to move forward with biannual HRAs for employees and spouses (alternating years).
The 3% rate differential is a difference of:
$20.44 per month for a single plan or $51.11 for a family plan on the HMO and Point of Service Health Plans
$245.28/$613.32 annual savings
$14.42 per month for a single plan or $36.04 for a family plan on the High Deductive Health Plan
$173.04/$432.48 annual savings
Notes: Figures are based on full time employment and based on 2025 health insurance rates. Base rates will change slightly for 2026.
What does this year’s process entail?
FOR THOSE WITHOUT A SPOUSE WHO COMPLETED THE HRA FOR 2025:

Your 3% rate differential will be applied for the 2025 and 2026 health insurance plan years.
FOR THOSE WITHOUT A SPOUSE WHO DID NOT COMPLETE THE HRA FOR 2025:
Please choose one of the following options:
Option 1: Complete a Wellness Visit at our MCPASD Employee Wellness Clinic and complete any recommended screenings. *This is the recommended option because we know the level of care you receive at the district clinic is so great, it will positively impact your health and well-being.
Wellness visits must be completed between 11/15/24 and 11/15/25
No forms need to be submitted by the employee for this option
- OR -
Option 2: Complete an annual physical with their Primary Care Provider (PCP), complete any recommended screenings and submit a Preventative Wellness Physician Verification Form completed by their Primary Care Provider (PCP)
Physicals and screenings must be completed between 11/15/24 and 11/15/25
Member is responsible for submitting the form to wellness@mcpasd.k12.wi.us
Form needs to be submitted by November 15, 2025
As a reminder, preventive physical exams from your primary care team are covered once per year. When scheduling your physical, ensure that it’s scheduled more than one year since your last physical to avoid additional charges.

You will receive a confirmation email when your 2025 HRA process is complete (note that MCPASD receives reports once a month, so your confirmation could take up to a month to receive).
If you choose to have a wellness exam at the MCPASD Employee Wellness Clinic, it DOES NOT count as your one covered, annual physical from your primary care team. Wellness Visits at the MCPASD Employee Wellness Clinic are coded differently than a PCP annual physical.
No medical information is shared with the district.
We strongly encourage scheduling appointments as soon as able to in order to ensure enough time to complete recommended screenings.
