While the day-to-day care and decisions that you make as a licensed clinician will remain unchanged, at times you may wish to receive the support or coordinated care of a clinicial team. This should go through the home health agency directly as Lifespan is not a healthcare provider. Clinicians are encouraged to reach out directly to home health agencies for urgent concerns, such as vital signs outside of parameters or critical medical decisions. This ensures a swift and targeted response to urgent clinical needs. The agency of record is listed both on the Lifespan Therapy app as well as the patient's EMR record.
Agency Affiliation
When contacting patients to schedule, it is importnat to remember that Lifespan is not the medical provider for these patients -- they are still under their agency of record. During these instances, it is crucial for clinicians to clearly identify themselves as representatives of the specific agency they are affiliated with. This prevents confusion and ensures the patient does not incorrectly believe you to be associated with a different home health agency. The agency of record is listed both on the Lifespan Therapy app as well as the patient's EMR record.
