To maintain compliance with local, state, & federal laws, Lifespan will not staff patients to clinicians with expired, non-compliant, or missing credentials. The agencies Lifespan works with expect to pass Joint Commission, state, and CHAP surveys in addition to Medicare and Medicaid accreditation. Additionally, a standard of care should be met for safely treating all patients. Below is more information on the acceptability and anticipated renewal timeframes for each credential.
Because these credentials take a bit more planning, HR will contact you 2 months ahead of expiration to ensure a plan is in place for the following credentials:
Your driver's license or Real ID may be from a state other than California but should be renewed according to state guidelines. It is recommended that you renew your driver's license as soon as you receive a renewal notice in the mail as processing can take several weeks. If you complete a legal name change, kindly update your driver's license and/or provide evidence of your name change (marriage certificate, court proceeding, etc.) A temporary permit is acceptable to see patients so long as it is still valid.
Basic Life Support (BLS) is preferred over a standard CPR court. However, if you take a standard CPR course it should be accredited by the American Heart Association. To locate an AHA accredited BLS course in your area, you may visit CPR.Heart.org Please note that CPR cards are typically valid for 2 years and, if no exact date is given, we will record the final day of your card's expiration month.
A physical is not the same as a checkup! We are happy to see that you are healthy, but please do not submit pap smear results or pages diagnostic testing. What the Department of Public Health wants to see is an annual, signed and dated statement from a physician advising you are cleared to work. It is recommended that you also have your doctor perform a TB symptom review or screening at this time. If you are changing insurances or your insurance only covers 1 visit per year (to the day), you may wish to plan your physical in advance. In a pinch, CVS Minute Clinics often accept walk-ins.
You should receive a notice in the mail to update your professional license. It may take time to receive your card in the mail, so if you have submitted the paperwork and payment you may simply notify HR. They will review the board website and confirm your license has been renewed and is active. Once you receive the paper copy in the mail, you may take a picture or scan this in and send it to HR.
TB testing methods will change depending on whether or not you have had TB in the past, as well as whether or not you are an at-risk demographic.
Initial Testing
If you have opted to receive a chest X-ray for TB in the past, typically because you have had TB or are part of an at-risk demographic, it is highly recommended that you not perform this annually due to radiation exposure. Annual chest X-rays are not recommended by OSHA or the CADPH. Instead, you may ask your physician to perform a TB screening during your annual physical or during a separate appointment. A blood test may also be performed to measure the occurrence of TB proteins in the blood.
If you have not had a positive TB test, you may still receive a chest X-ray, blood test, or simply perform a TST. During a TST, tuberculin is injected in to the forearm and the patient returns within 48 and 72 hours later to have the test read. If you obtain a TST, it is important that both the administered as well as test read date be accurately recorded.
Annual Statement of Clearance
A risk assessment should be performed by a healthcare worker annually. A risk assessment should include your full name, your date of birth, the date of your assessment, a statement of clearance, and a signature from the healthcare worker. This is different from your personal TB Questionnaire.
California Department of Public Health
Business licenses are required to operate as an independent contractor if required by your city of residence. To date, we are only aware of Mission Viejo and Running Springs not offering business licenses. San Diego, Irvine, Los Angeles (city), and most populated cities will require a business license/tax/certificate/occupancy license. These credentials take far less planning and can typically be obtained or completed same-day. Therefore, HR will contact you 1 month ahead of expiration for the following credentials:
A TB Questionnaire is a symptom screening performed by yourself. This may be quickly performed via this Docusign link and should have a date matching your most recent (current) TB test. It is necessary that you complete this self-screening in addition to any formal screenings or testing completed with a physician.
It is California state law that all drivers be insured. Your car insurance may be under a joint policy but should have your legal name listed as either a qualifying driver or primary insured. Because some insurances will only renew the day of, you may wish to inform the HR department if this is how your insurance operates. This will prevent future follow-up. If your insurer provides renewals in advance, you may send these cards as soon as you have them and HR will keep both the current and future copies on file.
Professional liability is necessary to protect yourself in the event of an emergency. Lifespan maintains liability insurance coverage, but as an independent contractor you must also cover your business activities. HPSO offers next day malpractice insurance.
