OASIS E is the version of OASIS (completed during a Start of Care OASIS, Discharge OASIS, Re-certification OASIS, Resumption of Care OASIS, or Transfer OASIS) that will go into effect January 1, 2023. OASIS itself is a dataset, or survey, used by CMS and researchers to determine payment and federal budgets, track a patient's progress throughout the care continuum, and assess health metrics. As federal budgets tighten, geriatric populations grow in size, and research solidifies the need to track new data or amend best practices, the OASIS is modified. This recent iteration includes many non-"m" questions, instead pulling questions from other levels of care for ease of tracking as patients move from the inpatient rehab setting, to SNFs, to home health, and finally to either hospice or outpatient care.

Replacing M0140 will be items A1005 and A1010 - ethnicity and race. For both, select all that apply.
If the patient declines to respond (Y), you cannot obtain this information from their record, guess, or ask anyone else - so selecting (Y) means only (Y) should be selected.
If the patient cannot respond (X), you can ask someone else or obtain this information from their record - so selecting (X) means you should also answer A, B, C, D, or E.
Completely new will be question A1110, language. This is a two-part question.
In part a. you will fill in the primary language. There is no list of acceptable languages, for instance, you may enter either Tagalog or Filipino.
In part b. you will answer if the patient wants or requires an interpreter.
Completely new will be question A1250, transportation. Select all that apply.
If the patient declines to respond (Y), you cannot obtain this information from their record, guess, or ask anyone else - so selecting (Y) means only (Y) should be selected.
If the patient cannot respond (X), you can ask someone else or obtain this information from their record - so selecting (X) means you should also answer A, B, or C.
Completely new will be questions A2121, A2122, A2123, and A2124 - asking if you have provided a medication list to the next provider and patient, as well as how this was performed. A reconciled medication list must always be provided to the patient.
A2121 - Provision of reconciled medication list to subsequent provider. This question will only appear if M2420 (discharge disposition) is in the community with formal assistive services or transferred to a non-institutional hospice. In this case, a reconciled medication list will always be provided to the subsequent provider by the home health agency, so you should always select (1 - Yes).
A2122 - Route of medication list transmission. Agencies will fax (D) or email (E) the medication list to the next provider.
A2123 - Provision of reconciled medication list to patient. This question will only appear if M2420 (discharge disposition) is in the community without formal assistive services or unknown due to moving. In this case, a reconciled medication list will always be provided to the subsequent provider by the Start of Care clinician, case manager, yourself, or the home health agency, so you should always select (1 - Yes).
A2124 - Route of medication list transmission. Yourself, the agency, or another clinician will provide the medication list to the patient. This is often performed both verbally (C) and on paper (D).
Completely new will be B0200, hearing, and replacing M1200 will be B1000. Use a combination of interview and observation to answer. These are not trick questions!
Completely new will be B1300, health literacy. This refers to an understanding of the healthcare reading material, not an ability to read, which was covered under vision.
Completely new will be the expanded cognition section consisting of C0100-C0500, the Brief Interview for Mental Status, BIMS interview, and C1310, signs/symptoms of delerium.
C0100: Can BIMS be conducted? This should be answered as yes (1) unless the patient is unintelligible.
C0200: Repeat three words. Ask the patient to repeat sock, blue, and bed. Record how m any of the three they are able to repeat.
C0300: Understand time. Ask the patient for the year, month, and week day, record how close they get.
C0400: Recall. Ask the patient to repeat the words that were asked earlier - sock, blue, and bed. Record if they get these correct or not.
C0500: Scoring. Add up the score to 0200, 0300, and 0400 - this should be between 00 and 15.
Replacing M1730, depression screening, will be D0150 and D0160, patient mood interview and scoring.
Completely new will be D0700, social isolation.
Replacing M1242, the frequency of pain interfering with activity, will be J0510, the effect of pain on sleep, J0520, the effect of pain on therapy, and J0530, the effect of pain on activity.
J0510: The effect of pain on sleep.
J0520: The effect of pain on therapy.
J0530: The effect of pain on activity.
Completely new will be K0520, nutritional approaches. You should check all that apply.
Do not miss therapeutic diet (D) which is common with diabetic, over/under weight patients, and those with heart conditions!
Completely new will be N0415, high risk drug classes. Check all that apply from the classes: antipsychotic, anticoagulant, antibiotic, opioid, antiplatelet, and hypoglycemic.

Completely new will be O0110, special treatments and procedures. You should check all special treatments that apply. Common treatments to be aware of for home health patients include oxygen therapy (C1 - C4) and dialysis (J1-J3).
