Ordering medical equipment can be a difficult process, so we have developed the form below to simplify things. Simply follow along and this form will inform you if the patient qualifies, according to Medicare guidelines, for the equipment being ordered. If they do, paperwork will be automatically generated and forwarded to the DME department for physician processing.
