It is the responsibility of each teacher to maintain a record of Continuing Education Units that a teacher may choose to use for renewal of a SD teaching certificate.  

You must keep track of this form for yourself.  The fine arts department will no longer maintain a list of dates, topics, and hours of professional learning.

All hours from SFSD professional development activities must be documented on this form.  Use a new form for each year.
