Please follow the withdrawal instructions below for your child(s) current school.
Only the student's legal guardian who enrolled the student can withdraw them.
Withdrawal requests for FVES students must be submitted through the official FOCUS withdrawal form in the parent portal account.
Log into your FLVS FOCUS parent portal account
Select the "FVES: Enrollment - Withdrawal Request (K-5) Form" from the Forms menu on the left-hand side of your home page
Complete all required fields on page 1 of the form and submit
Once the withdrawal has been processed, You will receive a withdrawal confirmation letter to the primary contact's email address.

Select the "FVMS: Enrollment - Withdrawal Request (6-8) Form" from the Forms menu on the left-hand side of your home page
Complete all required fields on page 1 of the form and submit
Once the withdrawal has been processed, You will receive a withdrawal confirmation letter to the primary contact's email address.

Select the "FVHS: Enrollment - Withdrawal Request (9-12) Form" from the Forms menu on the left-hand side of your home page
Complete all required fields on page 1 of the form and submit
Once the withdrawal has been processed, You will receive a withdrawal confirmation letter to the primary contact's email address.
If you plan to withdraw your child from our school to participate in the Family Empowerment Scholarship Program, please be advised that the Florida Department of Education now requires a standardized withdrawal form to be completed.
To begin the process:
Complete your portion of the form.
Submit the form to flvsk12records@flvs.net
Our school will then complete our section and return it to you for submission to your scholarship organization.
This form must be submitted before scholarship funding can be issued, as required by Rule 6A-6.0952, Florida Administrative Code.
