Intent to Return

The Intent to Return Form and Senior Exit Survey are Mandatory Compliance Forms 

When available, the FLVS FT Intent to Return Form will be on the Focus Parent Portal for K-11 students. 

The Senior Exit Survey will be on the Focus Student portal for 12th grade students. 

Start the form by clicking on the Green Button. Please respond to all questions on the form and select "Submit".

If you have any questions about completing the form, please contact your Enrollment Technician

If you have any questions about course or elective selections, please contact your student's School Counselor

These forms must be completed and submitted to regain access to all other areas of the portal. When mandatory forms are pending, the home screen will display a notification, as shown in the image below.

To ensure uninterrupted access to your FOCUS parent portal, please complete and submit the required forms promptly.
