How to Upload Immunization Records in FLVS FOCUS
Log In: Start by logging into your FLVS FOCUS parent portal account.
Select Your Student: Click the drop-down menu next to your student’s name in the left hand menu bar.
Navigate to Child Info: From the student's menu, choose “Child Info.”
Find Immunizations: Select “Immunizations” from the options.
Upload Your Document: Click into the field to add your document.
Select Your Student

3. Select "immunizations"
4. Upload your document
Email the enrollment team an updated copy of the student's FL DH680 or other Florida Department of Health vaccination record.
Emails should be sent to flvsk12records@flvs.net. Use the subject line "immunization records". Please include the student's full name and grade level in the body of the email.
