An initial referral should be deemed appropriate when:
Determined an educational need by the Campus Intervention Team (CIT)
Determined an educational need by the Campus 504 Committee
Parent Request
Teacher Request
Collection of Information prior to Consent for Initial Evaluation will be handled by the campus designated general education staff member. In many cases this is the Campus Counselor.
Required Information includes:
Home Language Survey (usually located in students' campus cumulative file)
Intervention Summary
RtI Meeting Documentation and Plan
504 Plan
LPAC Report
District and State Assessment Data (this can be collected by the assessment staff through Eduphoria)
Dated Parent Request letter (if applicable)
Campus Designee will:
Collect all requested Referral information (listed above)
Complete the Ferris ISD Evaluation Request Form
Keep a copy of all documentation
Provide referral packet information along with the Ferris ISD Evaluation Request Form to Central Office Special Education Department through district mail
Maintain a Referral Log with date provided to Central Office Special Education Department
For Parent Requests, the Assessment staff will:
Receive all information from the campus designee
Add student to eStar
Obtain consent from the parent
Provide referral packet information along with consent to Central Office Special Education Department
Central Office Special Education staff will:
Verify all needed information is received
Log in receipt/date of initial referral information
Enter student information in the Special Education Tracker for initial referral information
Scan all referral information for electronic access
Add student to eSpEd System
Send all appropriate scanned information to appropriate Assessment Team Members based on Area of Suspected Disability
Example:
If area of suspected disability is Speech Impairment only, initial referral information will be provided electronically to the Campus Speech Language Pathologist.
If the areas of suspected disability are Autism, all initial referral information will be provided electronically to the Campus Educational Diagnostician, Speech Pathologist and District LSSP.
Add the student into eSTAR using the "Add from List" option. Input the student's name, check the Add from list box and Add Student. (If you've added the student and are unable to see them in ARD forms, contact the SpEd department.)
