The reevaluation report must be completed by the due date determined on the REED. An ARD should be held immediately upon completion of the reevaluation (not to exceed 30 calendar days from the date of FIE completion).
Best practice is to review reevaluation reports at the annual ARD meeting making it the annual/reeval.

Document all attempts to obtain the parent information. A minimum of 3 efforts should be made before proceeding with prior year's information.

First solution will be to: Complete a REED/Evaluation (remember to review the guidance on the REED page, you may need to contact the SpEd Dept. to have this approved)
Next solution: Provide parent with a PWN stating we are ready, willing and able to test once we are provided access to the student. (contact the SpEd Dept. for approval)

You can start gathering data at the Annual ARD preceding the reeval due date if you so choose. Be sure you add this information to the student's archive manager for future reference.
Begin gathering data in August for semester one reevals and December for semester two reevals. You want to give yourself plenty of time to allow all involved to provide their portions. (OHI forms from the doctor, AI/VI, parent info, etc.)
Questions to ask/consider:
How badly did the student fail the vision test? If it was close, the nurse could retest and see if they can pass.
Does the student normally wear glasses and just didn't have them or wear them when they were being tested?
The nurse should have a form they complete when they do a vision test. We need to send this form home to the parent and ask the parent if they can take the student to get tested.
(If the parent says they cannot afford to take them to the eye doctor, we can work with the lions club and they will help us help the family get the student tested.)
If you are writing a fresh FIE, but using previous Cog/Achievement data, fill out the new disability report with the info from the old one, date it with the new FIE date and on the signature line, rather than signing it, put "information from FIE/disability report dated -----".
