Assessment Team members will receive initial referral packet from the Central Special Education Office electronically (or via the campus counselor for parent requests)
Assessment Team member (SLP or Educational Diagnostician) will schedule a time to meet/phone conference with parent/legal guardian within 10 days of receipt of the Initial Referral information to:
Provide Notice of Evaluation to Parent (make copy for SpEd folder and be sure to archive in eSpEd)
Obtain Informed Consent for Special Education Evaluation within 10 days upon receipt of referral information (be sure to archive in eSpEd)
Provide Parent Procedural Safeguards and receipt
Provide Parent Guide to the ARD Process and receipt
Complete the appropriate evaluation
Communicate with other Assessment Team Members concerning completion of evaluation
Complete the Full Individual Evaluation Report in eSpEd (be sure to archive in eSpEd)
Schedule ARD and place on ARD Tracker
Provide Notice of Evaluation to Parent (make copy for SpEd folder and be sure to archive in eSpEd)

Add initial evaluation to Initials Tracker
Send entire referral packet to SpEd Office so a folder can be created and information can be sent to needed team members
Complete the appropriate evaluation
Communicate with other Assessment Team Members concerning completion of evaluation
Complete the Full Individual Evaluation Report in eSpEd (be sure to archive in eSpEd)
Schedule ARD and place on ARD Tracker
Notice of Proposal to Evaluate
Speech Language Pathologist will complete Notice of Proposal to Evaluate in eSpEd for a child suspected of a Speech Impairment Only.
Educational Diagnostician will complete Notice of Proposal to Evaluate in eSpEd for children suspected of all other areas of suspected disabilities.
eSTAR Consolidation of Important Dates
Must be completed immediately upon receipt of consent to evaluate.
