Calendar Activity Request

In order to have an activity added to the master calendar you must:

  • Fill out a Calendar Request Form. Found on Google Drive.

  • Get the appropriate signatures.

  • Submit this form to Mrs. Walker for final approval. Once the activity is approved, the activity will go on the official master calendar.

Field Trips

Required and non-required field trips MUST be requested at least two (2) weeks prior to the trip using the Educational Journey Trip Request found on the CMS intranet site

In addition to requesting your trip over the intranet, all teachers must submit the following paperwork which will be reviewed and approved by the Instructional Leadership Team (ILT):

  • Trip Description

  • Itinerary

  • Learning Opportunities/Relevance to CMS Curriculum

  • Roster of students

  • Roster of adult chaperones (Note: All non-CMS employee chaperones MUST have level 4 volunteer clearance)

  • Request for absence for any staff member attending trip, and the staff member must submit their absence via SmartFind Express. A request for absence for a field trip does not excuse a staff member from reporting to work or departing from work outside of the normal schedule. Staff members are required to work their normal duty day and depart campus at the scheduled time of the trip, and resume their normal workday upon return to campus.

Field trips cannot be approved or considered until all the above paperwork/documentation has been submitted.


Overnight trips must, in addition to the above required forms, include the following:

  • Checklist for CMS-Sponsored overnight travel (5302)

  • Notice, Permission, and Release for Overnight trips. Both pages. (5300)

  • If using a charter bus, Addendum to Contract for Travel Related Services (5301) and a signed contract from the charter bus company.

THE STUDENTS ARE RESPONSIBLE FOR DOCUMENTING THE ABSENCE IN ADVANCE WITH ALL THEIR TEACHERS.

Volunteers

A priority for CMS’s volunteer program is to ensure our students’ safety in the school environment. Volunteer screening is part of this effort. Screening starts with all volunteers registering to volunteer online at http://volunteer.cms.k12.nc.us. Additional screening is dependent on the assigned task, the degree of supervision of the volunteer, and the nature of the volunteer’s contact with the students. The level of volunteer screening will increase as a volunteer’s contact with a student increases and the degree of staff supervision decreases.
