The mandatory forms for the 2023 Senior class trip have been posted to the PARENT PORTAL IN GENESIS

These forms must be completed for your child to travel on the Senior Class trip.

The forms are labeled:

1. East Senior Trip Contact and Permission Form

2. East Overnight Field Trip Medical Form 

*For this form, if you list that your child takes prescription medication, 

we will need you to also fill out a Medication Authorization Form. 

**The Medication Authorization Form is hyperlinked in the Overnight Medical form 

and must be printed out, signed by the child's doctor, and returned to Ms. Barr in A015 (Activities)
