General Tips
Make it eye-catching! You need to grab their attention within the first 30 seconds. Employers are busy and first impressions matter a lot.
Include a CV/Resume but not necessarily on the front page
Task 1 - Create a site for your work
The easiest way to do this is to head over to google sites and create a new site for your work. If you want to use this site to showcase your work in the future then consider making the site with your personal email rather than school account.

Task 2 - Identify your skills
Identify your best work!
This could be:
Stuff you have made
Games
Models
Art
Music
3D Manufacturing
Writing
Scripts you have written. If you are interest in programming including a profile like CodeinGame is useful. DO NOT include screenshots of large blocks of code noone wants to read that. If you have a GITHub page include this.
Competitions you have been involved in
Start compiling these by collecting images, scripts, competition pages etc.
If you are in game design upload some of your games to itch.io so that people can play your games. Again uploading them to an account linked to your personal email might be the best idea if you want to use it professionally.
Task 3 - Start assembling you site
Go and have a look at some other portfolios and apply the ideas to yours.

Step 4 - Get feedback
Sorry for banging on about feedback but your first try at something will almost defiantly not be your best. Ask Someone else what they think and take that on board.
