Even though many precautions are taken to reduce the risk in workplaces, incidents can still occur. If you see any potential hazards it is your duty to notify an appropriate person such as a supervisor or manager, HSR (Health and Safety Representative) or HSC (Health and Safety Committee) member to ensure that the hazard is dealt with at the earliest possible time. If an incident does occur then reporting it is also manditory, an incident being both when someone has been injured and when there has been a 'near miss'.
You can can report a hazard or an incident in a variety of ways including formal or informal and verbal or written. An example of a formal, written report is an incident report that with be an official document created by the workplace. This is required for incidents and near misses. Informal verbal reporting may be just letting your HSR know that there is a small hazard that needs attention.

Everyone in a workplace is required to be part of picture in keeping everyone safe. This means that if you see something that is a hazard it is your responsibility to either deal with it if it is within your area of responsibility or expertise, or report it to the appropriate person. If we are not ALL looking out for hazards and dealing with them/reporting them in a timely manner then we put others at our workplace in danger.
Areas that should be reported can be risks either physical or mental and can have an immediate impace of long term effects. They include the following:
WHS issues and concerns
Workplace Hazzards
Unsafe Work Practices
Breaches of health, safety and securityÂ
Workplace bullying and harrassment
Discuss with the class examples of each of these areas.
After any incident, regardless of whether an injury occured or whether it was classed as a 'near miss', an incident form must be filled out and given to the approriate person.
ACTIVITY
Attched left is the current incident report form for our school. In groups of 3-5 you are going to create a scenario, typical of the Entertainment Industry and act it out for the other groups. Each student will then write up an incident report on the scenario and submit it to Google Classroom for feedback.
