EPA ID number is needed for all hazardous waste generators participating in consolidated manifesting hazardous waste.

2. ​​​​​Create an eVQ Account to keep your ID number active 

Hazardous Materials New Business Registration 

Companies that generate hazardous waste must complete and submit a Hazardous Waste Generator Application.  Follow these links for additional information and application forms: 

1. Create a CERS Account 

2. Complete Facility Information section in CERS.

3. Attach Hazardous Waste Generator Application to Miscellaneous State-Required Documents.

4. Submit

Medical Waste Generator Permit 

​​​​​​​Companies that generate any quantity of biohazardous waste must complete and submit a Medical Waste Permit Application.  Follow these links for additional information and application forms:
