EH&S Incident Reporting

It is vital that all accidents, injuries, and near misses are reported in a timely manner.  Employees, visitor, contractors, and members are responsible for reporting unsafe conditions, accidents and injuries, as well as security concerns, including reporting verbal and/or physical threats to ensure the appropriate investigation and corrective/preventative action is implemented to prevent a reoccurrence.

Whenever an accident, injury, or near-miss has occurred, EHS & Compliance and the Safety Officer, or designee will investigate to determine the root cause of the unsafe condition or unsafe act, and analysis of facts (who, what, when, where, and why) aimed at determining how it can be prevented in the future.  All findings and corrective and preventative actions will be documented and corrected per the Corrective and Preventative Action Procedure.
