A Certificate of Insurance is a standard form issued by an insurance company evidencing the insurance information (including policy limits and types of insurance) of its policyholders. Insurance is necessary to cover any claims or losses for which the vendor may be responsible.
The following minimum Commercial General Liability (CGL) insurance standards shall apply to all vendors working at Bonneville Labs. If Bonneville Labs determines that the work represents an unusual or exceptional risk, then it may require additional insurance.
$1,000,000 General Aggregate Limit;
$1,000,000 Each Occurrence Limit;
$2,000,000 Products-Completed Operations Aggregate Limit;
$1,000,000 Umbrella/Excess; $1 Million Occurrence/ $1 Million Aggregate
$1,000,000 Workers Compensation; Statutory Limits + $1 Million Employers’ Liability for Each Accident/Disease – Ea. Employee/Disease – Policy Limit
$1,000,000 Business automobile liability insurance, including non-owned and hired automobile coverage, with a combined single limit
All operations for the named insured performed for Healthpeak OP, LLC c/o Healthpeak Properties, Inc. ,Healthpeak OP, LLC c/o Healthpeak Properties, Inc. are included as additional insured for ongoing and completed operations for general and automobile liability. Waiver of Subrogation applies in favor of Healthpeak OP, LLC c/o Healthpeak Properties, Inc.for general, automobile, and workers compensation and employee’s liability policies. Excess/Umbrella liability policy follows form over general liability, automobile liability, and workers compensation and employee’s liability policies
