Beneficiary Designation
Employees hired to work at least 30 hours per week.
Choose 1x, 2x, 3x, 4x, or 5x annual salary, up to a $500,000 maximum.
If under age 70, you may elect up to $200,000 without medical questions (Guaranteed Issue).
At age 70 and over, coverage reduces to 25%.
Employees working at least 30 hours per week are automatically covered by Basic Life and Accidental Death & Dismemberment (AD&D) Insurance through The Standard — at no cost to the employee.
This coverage provides valuable financial protection for your beneficiaries in the event of your death and additional benefits if death or injury occurs due to an accident.
Basic Life Benefit: 1× annual earnings, up to a maximum of $300,000
Basic AD&D Benefit: Equal to your Basic Life amount
Option 1: $10,000 (spouse) / $5,000 (child)
Option 2: $20,000 (spouse) / $10,000 (child)
*A child cannot be insured by more than one employee.
100% employee-paid. Rates are based on age band and elected amount.

Frequently Asked Questions (FAQ)
Late Enrollee (missed new-hire window): Application and Evidence of Insurability (EOI) required.
Effective date: the 1st of the month following the month EOI is approved by The Standard.
If approval occurs on the 1st, coverage is effective that day.
Increase Coverage: New application and EOI required.
Effective date: the 1st of the month following the month EOI is approved (or that day if approval is on the 1st).
Decrease Coverage: New application required (no EOI).
Effective date: the 1st of the calendar month after HR receives your application.
If received on the 1st, the decrease is effective that day.
A short health questionnaire (and sometimes a physical exam) that The Standard uses to approve coverage amounts over the guaranteed issue.
Submit a new Beneficiary Designation form to HR at any time.
