Transporting Students

If you are transporting students each  of the forms below must be completed:  

• • Motor Vehicle Records Authorization Form (One Time)

  • Vehicle and Operator Requirements - Exhibit 1

  • School-Owned Vehicles - Exhibit 2 (District Staff Only)

  • Privately Owned Vehicles - Exhibit 3 (MUST include proof of insurance that shows dollar amount coverage)
