Employees working 30 hours or more per week are eligible to enroll in the District’s health insurance plan.
Dependent Eligibility:
Dependents are eligible to remain on the plan through the last day of the month in which they turn age 26.

You’ll need your group number or member ID, which can be found on your Dean insurance card.

Go to DeanCare.com and select Find a Doctor.
Choose Commercial HMO/POS Insurance when prompted.
You can search by specialty, or select a provider from the preloaded dropdown list.

Changes due to a qualifying event must be made within 30 days of the event.
Otherwise, changes can only be made during the annual open enrollment period.
The District’s plan includes:

Yes, Dean Health Plan requires members to select a primary care provider (PCP) who coordinates your care and provides referrals when needed. You can change your PCP anytime by logging in to your Dean online account.
Yes. Each year, members must meet a plan deductible before the plan begins sharing costs. You can view your deductible and coinsurance levels in the Summary of Benefits and Coverage (SBC) available above.
You’ll receive a Dean Health Plan ID card by mail. You can also access a digital version anytime by logging in to your Dean/Medica member portal or downloading the Dean Health Plan mobile app.
Yes! Dean’s Living Healthy Rewards program allows you to earn points for activities like preventive visits, fitness tracking, and health challenges. Points can be redeemed for gift cards.
