Readmissions
General Reminders
IndD Design students must file for readmission if they fall under any of the following and intend to enroll in the upcoming semester:
Returning after an approved Leave of Absence (LOA).
Tagged as AWOL (Absent Without Leave) in CRS
Deadlines for readmission appeals are not listed in the UP academic calendar, as they vary by college. For CFA programs (including IndD), announcements are posted on the official UPCFA Admin Facebook page. Check it regularly and plan ahead.
It is strongly advised to begin your readmission process well before the registration period to avoid delays in enrollment.
All requirements must be submitted before the end of the registration period of the semester you plan to re-enroll in, especially if you intend to take courses.
Incomplete submissions will not be processed
Readmission from LOA
Prepare the following documents. All information provided must be true and accurate, as it is subject to verification.

Appeal Letter
Address the letter to: Student Progress and Admission Committee
Include the following details:
Your student number
Degree program
First year and semester of entry into the program
Contact information
Clearly state the reason(s) for taking LOA and describe any activities undertaken during your leave.
Include your signature.
Request your Program/Research Adviser’s signature as proof of endorsement, placed under the line: “Endorsed by:”
Save your file using this filename format:
(Reason for Appeal) – (Surname) (StudentNumber).pdf
e.g. Appeal for Readmission from LOA – Dela Cruz 202112345.pdf

True Copy of Graces (TCG)
Request your TCG from Ms. Grace at mlrecto@up.edu.ph

Accomplished Return from Leave of Absence (LOA) Form

Filled-Out Student Directory with Your Photo

Medical Certificate from UP Diliman Health Service
Required only if:
Your LOA was due to health reasons (regardless of the number of semesters); or
You were on LOA for at least 2 semesters

Relevant Supporting Documents
Attach documents that support your stated reason for taking LOA (e.g. Medical Certificate, Certificate of Employment, etc.)
Email all required documents to Ms. Cerro at jacerro@up.edu.ph for committee evaluation.
Wait for the email from CFA regarding the result of your appeal and the status of your readmission. This will be sent after your submitted documents have been evaluated and verified to be in order.
Readmission from AWOL
Prepare the following documents. All information provided must be true and accurate, as it is subject to verification.

Clearly explain why you were tagged as AWOL, including any activities undertaken during the term(s) you were not enrolled.

Proposed Program of Study / Updated Curriculum Checklist
Required only if you exceeded the Maximum Residency Rule (MRR).
Include the following details:
All remaining coursework
Any deficiencies due to previously disapproved appeals
Any additional “penalty” courses, if applicable
Have this reviewed and verified by your Program Adviser

Relevant Supporting Documents
Attach supporting documents that justify your AWOL status (e.g. Medical Certificate, Certificate of Employment, etc.)
Email all required documents to Ms. Cerro at jacerro@up.edu.ph for committee evaluation.
Wait for the email from CFA regarding the result of your appeal.
If your appeal is approved by the Student Progress and Admission Committee, you will be asked to submit the following documents to complete your readmission:

Medical Certificate from UP Diliman Health Service
Required only if:
You were on AWOL for more than one sem; or
You were on LOA for one semester and AWOL in the next

Certification of Non-enrollment
Required only if you were on AWOL for 3 semesters.

Notarized Affidavit of Non-enrollment
Required only if you were on AWOL for 4 or more semesters.
Submit the next set of required documents to the contact person indicated by the Committee.
Wait for confirmation from CFA regarding the status of your readmission. This will be sent after your documents have been reviewed and verified.
