Understanding INC and 4.00 Grades
Managing a 4.00 Grade
A grade of 4.00 is converted to 5.00 if it is not removed through re-examination or if you do not re-enroll in the course within one year.

To earn credit afterward, you must repeat and pass the course.

Both the 5.00 and the new grade will be included in the GWA computation.
If you re-enroll in the course within one year of receiving a 4.00, the grade remains 4.00 and is not converted to 5.00, even after the deadline.

In this case, both the 4.00 and the re-enrolled grade are counted in your GWA.
If you receive a 4.00 in the first part of a two-semester course (a sequence of two courses taken over two consecutive semesters), you may still enroll in the second part.
If you pass the second semester within the same academic year, the 4.00 is automatically converted to 3.00.
If you fail, the 4.00 becomes 5.00.
If you are not enrolled in any subject but want to remove a 4.00 grade, you must enroll for residence during the semester to take the removal exam.

A removal fee may apply.
A grade of 4.00 appears on your Transcript of Records (TOR) and is included in the GWA until it is removed or the one-year deadline lapses.

Once removed, only the final grade (3.00 or 5.00) is counted in the GWA, but the original 4.00 remains on your TOR.

Removal Period Reminders
A grade of 4.00 received after completing an INC must be removed within the remaining time allowed for the original INC grade removal period.
A grade of 4.00 may no longer be removed or improved after the 3rd regular removal period immediately following the semester or term in which it was incurred (equivalent to one academic year).
A regular removal period for 4.00 grades is held before the start of each semester.
Refer to the UP Diliman Academic Calendar for official deadlines and schedules.
Special removal schedules, such as exams held outside the regular period, may be arranged with the approval of the academic unit or college and are subject to any applicable removal fees.

Processing your Completion / Removal Permit
Download the UP Form 25 “Permit for Completion/Examination/Removal” form or request a copy from the Student Records Section of CFA.
Email Ms. Grace Recto at mlrecto@up.edu.ph to request your Statement of Account (SOA).

Wait for the billing statement/SOA before proceeding with payment.

Then, fill out the payment slip.
Permit for Completion of INC – No transaction fee
Permit for Examination/Removal – ₱20.00 per course/subject
After receiving your SOA, pay the appropriate fee.
Email your proof of payment (receipt/confirmation) and the payment slip to the UPD Cash Office at cashoffice.upd@up.edu.ph.
Fill out the UP Form 25 completely before your scheduled completion/removal date.

Request approval from the College Secretary via collegesecretary.upd@up.edu.ph.
Once approved by the College Secretary, send the completed form to your Instructor for their signature.

The Instructor will return the signed form to you and submit your grade through the CRS grade completion/removal module.

Policy Reminder: Students must be enrolled during the term of completion/removal for the grade submission to be officially submitted.
Email the fully signed UP Form 25 (with Instructor’s signature) to the CFA Student Records Evaluator (SRE) at CFASRE.upd@up.edu.ph.
The Instructor will generate and submit the Official Report of Grades for Completion/Removal to the SRE.

The SRE will then forward the physical copy to the OUR – Records Section.
