Company Goal
Our goal at Source One Staffing is to assure that our customers and supervisors are familiar with the procedures to follow in the event of an injury. The sooner the injury is reported to Source One, the better equipped we can be in order to help our employees.
You, the customer, are a very important part in the process of injury prevention as well as the investigation following an injury. Source One Staffing requires all customers to follow all local, State and Federal laws and guidelines for work-related injuries and injury reporting. Source One also requests that if any customer sees one of Source One’s employees not acting in accordance with Our policy or acting in an unsafe manner, that you report that employee’s behavior immediately to a Source One Staffing representative. As always, if you have any questions, please feel free to reach out to a representative of Source One Staffing for guidance or clarification.
These are the general safety rules to provide to our employees
· Report an injury to your supervisor immediately and to Source One as soon as practical.
· Report any unsafe condition immediately to your supervisor
· Being under the influence of alcohol or any drug, illegal or prescribed, is not permitted. Any employee under the influence of alcohol or drugs will not be permitted to work.
· Appropriate clothing and footwear must be worn at all times while on the job site.
· No employee should perform any task unless trained to do so, and are aware of the hazards associated with the task.
· Learn safe work practices. When in doubt about performing a task safely contact your supervisor for instruction and training.
· Never remove or bypass any safety device.
· Do not approach operating machinery from the blind side. Let the operator see you.
· Learn where fire extinguishers, first aid kits and emergency exits are located.
· Practice good housekeeping by maintaining a clean and clutter free work environment.
· Be alert to hazards that could affect you and your coworkers.
· Obey all safety signs and tags.
· Always perform your assigned task(s) in a safe manner and as you are trained. Do not take shortcuts.
Procedures to follow when a work related accident occurs
After a Source One employee reports an accident to a supervisor:
1. Determine the extent of the injury and access what, if any, medical attention is required
2. The employee or Client Company must notify Source One immediately.
If the injury is not serious, ask the employee if he/she can come to the Source One office that the person was assigned from.
If this is a major injury and the employee feels that he/she needs medical attention ASAP, he/she should go to the emergency room.
If the injury happens after Source One business hours, please call our after business hours phone # and the representative will help you through the correct process.

After evaluating the injury to determine the extent, if the injury is considered minor and non-life threatening, the employee is to determine if he/she would like medical attention. There are occasions when the employee decides not to seek medical attention because the injury does not require it.
If the employee requests medical attention, and the employee is deemed able, please send the employee to one of our clinics. The locations of the clinics are on the following page(s).
If it is determined that the employee cannot transport themselves without risking further injury to either themselves or the general public, please contact medical transportation for the employee.

If the injury occurs after hours and the employee feels that they can wait until the next day to get medical attention, he/she should call the Source One office first thing in the morning to inform us of the injury.
If the employee feels that they need medical attention ASAP and the injury occurs after hours, he/she should go to the emergency room.
After any injury, the attached forms must be completed and copies sent to Source One Staffing. Where appropriate, Spanish versions of the forms are also attached.
· Employee Accident / Injury Report (to be filled out by employee)
· Foreman / Supervisor Report of Accident / Injury (to be filled out by Customer)
· Witness Form (One form to be filled out by each witness to the event(s) leading to the injury / accident)
· Refuse Medical Treatment Form (to be filled out by the employee in the event that he/she refuses any professional medical treatment)
It is extremely important to conduct a timely and thorough investigation of any injury or accident. It is also extremely important to interview as many witnesses as possible as this will determine possible root causes of the accident or injury and ultimately determine the compensability of the claim. This may also determine if any corrective action needs to be taken or if any procedure(s) need to be reviewed and / or revised.
