EMERGENCY MEDICAL INSTRUCTIONS

Supervisors should send a Treatment Order/Authorization Form with the employee along with a First Report of Injury Form if possible. The First Report of Injury Form should be immediately completed by the supervisor and submitted to HR.
ATTENTION ALL SUPERVISORS AND MANAGEMENT PERSONNEL
The following procedures are to be followed when an on-the-job injury occurs:
Once notified of an injury, a supervisor should make an assessment and begin first aid if necessary.
▲ For severe situations that require it, call 911 right away.
▲ Do what you can to stabilize the employee’s injury.
▲ Ask the employee what happened, find out if there were any witnesses and then fill out the First Report of Injury Form. Send a copy of the report with the employee to the clinic and give the original form to Human Resources.
▲ If an ADMO employee is injured, but may safely drive themselves to the clinic, they may do so. If it is unsafe for them to drive, the supervisor should call for an ambulance.
▲ If the employee refuses treatment, in addition to the required injury report forms, supervisors should have the employee sign a waiver of treatment form.
FOR INJURIES INVOLVING EMPLOYEES FROM A STAFFING AGENCY
For injuries involving an employee that is from a staffing agency, immediately stabilize and treat the injury with first aid and contact the staffing agency for further instructions.
If the injury is severe or life-threatening, immediately call 9-1-1 and request an ambulance.
