Vaccination Locations

All vaccinations are by appointment only at more than 900 locations in the state’s provider network.

Make an appointment with one of the providers below. Residents who need assistance or lack online access can call the toll-free IDPH hotline at 833-621-1284 to book an appointment.



Here are some helpful reminders to help you successfully secure your vaccination appointment:

• Please bring documentation to your appointment to show that you are eligible for vaccine as part of a priority group. Providers may NOT ask individuals to provide documentation regarding their immigration status.

• Visit the Statewide Vaccination Locations page for additional locations open to all Illinois residents. The map below will help find the closest location to you.

• As the federal government distributes additional vaccine doses, additional appointments will become available. New sites and pharmacies are being added daily. Please check back regularly for available appointments.
