Capstone Update (2015):
Some aspects of the project has changed and merged into the Crossover Youth Practice Model (CYPM) work. CYPM will be piloted in two communities as a larger overarching initiative. As part of the planning process, teams and partners will map out 40 youth's life history, review their system involvement, and identify any missed opportunities for agencies to provide support. Given the high number of assessment tools already in place within Children's Division, the team will not develop a separate risk assessment tool. However, the team plans to take an inventory of the information related to risk factors for crossing over from child welfare to juvenile justice
