Dental Examinations

All Kindergarten and Second Grade Students are required to provide proof of dental exam no later than May 15, 2021. Exams completed from 11/15/2019-present are acceptable.

If you have not already made an appointment, please do so ASAP to ensure you're able to get the exam completed before the deadline. Please make sure to fill out the top part of the form before you return it to school.
