Allergy and Asthma Information
For those students with food allergies, a new Food Allergy & Anaphylaxis Emergency Care Plan is due on the first day of school of each school year; please submit this form along with your child’s emergency medication. The updated forms are required for us to update your child's 504 plan. Please make sure to schedule your yearly appointment early so it will be completed before the first day of school.

D39 requires a current Asthma Action Plan for those students who keep a bronchodilator at school. A sample plan for home and school can be found below.
