In May 2016 The Telegraph in the UK featured an article about Mary Killen who answered many questions on etiquette.
There are a range of topics covered, and there is also a questionnaire in the article - "Mary Killen's Modern Manners Quiz', which you will find further on down the page. Once you enter your response to a question, the quiz shows you the correct answer immediately.
The other website you could look at is from The Online Magazine About Etiquette. They have a number to choose from but here
