Dropped off at the main office of your elementary school from 9:00 AM - 2:00 PM
Copied and mailed to the school (see school address below)
Scan and email them to the school secretary
If unable to drop off at main office of your elementary school, bring to Superintendent’s Office at Town Hall (Room 204) Monday and Wednesday from 7:00 a.m. – 5:00 p.m., Tuesday from 7:00 a.m. – 8:00 p.m. or Friday from 7:00 a.m. – 4:00 p.m.

Complete the online pre-registration form**
Present the following required registration documents to the neighborhood school during registration week:
proof of residency
health history and health records including immunization records
Original birth certificate with seal
*Kindergarten age-qualified children are those that have turned five (5) years old before September 1 of the school year for which they are pre-registering.
** If your child is currently enrolled or receiving special education services thorough Daniel Feeney Preschool Center, you do not need to submit the online registration form, birth certificate, or proof of residency. However, you will need to provide the elementary school with a copy of your child’s latest physical.

Parent/guardian phone numbers, including home, cell, and work
Parent/guardian email address
At least two alternate emergency contacts (authorized to pick up student) including name and phone number
The name and address of the previous school attended, if applicable
Health Insurance policy number if covered by a public health insurer
Original birth certificate with seal
Proof of residency
Most recent student physical form
Health Insurance - provider name and insurance number
