Health Requirements
All new students must provide the following health information:
A record of the following Immunizations must be provided PRIOR to entry :
5 doses of DTaP/DPT
4 doses of Polio
3 doses of Hepatitis B
2 doses of MMR
2 doses of Varicella OR Record of Chicken Pox Disease with written verification by the child’s physician
2. Record of a Physical Exam dated within one year prior to
entry to the Walpole Public Schools, or within 30 days of
School entry.
3. A completed Health History form.
4. A Tuberculosis test for students entering from areas of the
world where the disease is found at increased levels. This
is determined on a case by case basis.
5. Students entering Kindergarten have additional
requirements. Please see the Kindergarten registration
information.
