Fill Out Student Info & Team Members
Record student name, grade, date of meeting, and all staff or caregivers present.
Describe the Problem Behavior
Briefly and objectively describe the behavior of concern (e.g., “Student leaves class without permission 3+ times per day”).
Define Target Behaviors
Identify specific behaviors to decrease and replacement behaviors to increase.
Use observable, measurable language.
Identify the Function of Behavior
Based on FBA or team observation, check what the student is likely trying to achieve (attention, escape, sensory, etc.).
Write a simple hypothesis (e.g., “When given independent work, student throws items to escape the task.”)
Design PBIS-Aligned Interventions
Select supports in four areas:
Environment
Teaching replacement behavior
Positive reinforcement
Response strategies
Customize to what works for the student (based on strengths, triggers, and needs).
Determine Data Collection Methods
Choose how progress will be measured (e.g., CICO, behavior log, parent contact).
Assign who will collect and review the data.
Share the Plan
Click Share and set permissions (e.g., "Anyone with the link can view").
Send a copy to all team members and the student’s family.
Put the Plan in Action
Begin using the intervention strategies the very next school day.
Make sure all involved staff are aware of their roles (especially teachers and aides).
Monitor the Data
Collect and review behavior data as outlined in the plan.
Hold brief weekly check-ins if needed.
Hold Review Meetings
Use the “Review” section in the plan to document follow-up meetings.
Discuss progress, modify supports, or intensify services if necessary.
