Parents Night Out
ALL volunteers that will work at the school while students are present MUST pass a Live-Scan background check. These checks are not transferable between locations, so even if you have done one before for another organization you must do a new one for Mistwood. Live Scans cost $30-$49, and that amount is able to be reimbursed to the families service hours (3-5 hours).
Step 1: Come to Mistwood and ask the staff for a “Request for Live Scan Service" form. You will also need a legal ID and a way to pay for the live scan.
Step 2: Take the Live Scan form to one of the agencies listed: Humboldt County Office of Education has a list of locals you can get the live scan done.
If you are planning to drive students, other then your own you will also need to complete/submit the following to Mistwood:
Copy of your Drivers License
Copy of your Car Insurance
