it's not just you. The new mode is really hard.
It’s not “endless” per se - the run ends after you fight Leshy. It’s more of a challenge mode. However, it’s endless in that you’ll unlock cards, challenges, and starter decks as you progress, so it makes it more engaging to play repeatedly.
You have to beat Leshy with challenges active.
The first two Challenge Levels require 5 points and 15 points, respectively; from then on, every challenge level requires (10 × the challenge level) points to clear it.
Bragging rights. (No, you can only clear one challenge level at a time.)
No - it’s permanent within a run, but its stats reset between runs.
No again (technically, the default ones still appear in Leshy’s fight, but you cannot create them.)

The map order is random and they can occur in any order, though Leshy will always be last.
Campfires are now limited to two buffs. The first is guaranteed; the second has a 50% chance of the card being eaten.
Survivors can still be killed, but killing them is not always recommended - see the section on deck downsizing in Tips.

The limit is 7 total sigils: 2 innate, 4 added, and 1 totem.
You can only transfer sigils to a card once, so you can have either 1 or 2 sigils added without the Mycologist.
You can add up to 4 sigils (via Mycologist), but any sigils after that will just disappear.

Instead of Starvation when you run out of cards to draw, the moon gains attack each turn. This is called “Moon’s Mercy” and is present in the base game. See the Game Mechanics > The Moon section for more details.

Yes! You should be able to get the Devil’s Play achievement, though a couple people have had problems with it. There are also new achievements specific to Kaycee’s Mod added in V0.34.
