EMS Band Polo Shirt Order Form

wELCOME TO sCHOOL yEAR!

As a member of the 7th and 8th Grade Band, your son/daughter will be in need of their concert uniform. Uniforms are worn at all concerts and public events. Uniforms for the 7th and 8th Grade Band consist of our RED polo shirt, black dress pants, black shoes and socks. Members of the band will need to purchase the RED polo shirt and provide their own black pants, shoes and socks. This uniform will be the same for all concerts this year.

*** PLEASE NOTE ... this is the same RED SHIRT as last year, so you do not need to order a 2nd shirt unless last years shirt is lost, or too small. ***

Our EMS BAND PaySchool option is our payment method for any band-related expenses that may arise during the school year. This streamlined approach is quick, convenient, and eliminates the need for your student to deliver payments to Mr. Hutchison.

#2 Enter First and Last Name of the Student

Certainly, if you prefer to pay by check, you are more than welcome to do so.

Here are the steps for paying by check:

1. Make the check payable to Grand Blanc East Middle School.

2. Place the check in a sealed envelope.

3. Write the following information on the envelope:

- Student's First and Last Name

- Amount for Polo Shirt

4. Kindly instruct your student to HAND THE ENVELOPE TO ME. :).
